Minnesota Department of Revenue Asking Nonprofits to Assist in Updating Exempt Status Information

The Minnesota Department of Revenue (DOR) is in the process of updating its system to better serve nonprofit organizations that are exempt from paying sales tax. As part of this update, they need to confirm your organization’s current name, address, and other information about your organization.

The DOR will be sending out a letter and form requesting updated information if you have Minnesota sales tax exempt status. These letters will be automatically mailed out in batches over a period of time in the near future. If you have completed the form below and sent it to the DOR, you can disregard the letter you received in the mail.

You may also voluntarily complete the form prior to receiving the letter. Please complete the Nonprofit Information Form following the instructions below.

What do you need to do to update your information?

Complete the following information on the form:

  • Enter your organization’s current name and address. Indicate if the name or address has changed from what is listed on the letter by placing an “X” in the box on the form.
  • If your organization’s name or address has changed, enter the previous name and address.
  • Enter your Federal Employer Identification Number (FEIN).
  • Enter your seven-digit Minnesota Tax Identification Number.
  • Indicate your organization type by placing an “X” in the appropriate box and provide all of the required information listed on the form.

If you are no longer in business, how do you complete this form?

  • Indicate your organization type by placing an “X” in the appropriate box.
  • Answer “No” to the first question to indicate you’re no longer in business.
  • It is not necessary to provide any other information.
  • Mail or email the form to us at one of the addresses provided above.
