Organization Summary:

 MoveFwd has served young people and their families in the western suburbs of Hennepin County for almost 30 years.  Our mission is to partner with young people to achieve goals that are important and meaningful for them.  We offer housing support for homeless youth or those at risk for homelessness, mental health therapy, a drop in center, and a transitional housing program.  Our services are available for young people ages 12-25.

The MoveFwd Transitional Living Program provides housing and support services to young adults who are exiting homelessness.  The case managers provide individualized and specialized support to participants focusing on obtaining meaningful employment, financial literacy, meeting program and landlord expectations, maintaining housing and increasing academic achievement when applicable.

Hours: 40 hrs per week M-F 
 

Primary Duties:

 Provide coordination of case management services to young adults in the Transitional Living Program and additional participants as needed.

Conduct assessments with participants to determine goals, barriers, resource needs and areas of opportunity , acting as a broker and advocate for community resources.

Maintain consistent relationships and home visits with the participants on a weekly basis

Act as a liaison between participants and landlords related to tenancy issues and/or advocacy needs

Assist participants in the skills needed to live independently

Facilitate a monthly support/independent living group for all TLP participants

Provide guidance in housing search process and independent living skills while in the program.

Provide transportation for participants to necessary community resources and group meetings


Experience:

BA degree in Social Work or Youth Development strongly preferred, related fields will be considered

2 years experience working with youth who have experienced homelessness and/or opportunity youth required

Experience completing assessments/coordinated entry preferred

Experience completing goal plans preferred

Ability to transport participants in own vehicle, good driving record required and car insurance/drivers license up to date

Ability to lift up to 15 pounds

Flexibility in scheduling appointments with participants and community partners

Ability to work evening hours each month to accommodate facilitation of the monthly group
